This person in this role will be expected to provide care invarious residential settings including private homes assisted living and skilled nursing facilities.

Job Summary:
TheHospice Admission Nurseis responsible for conducting comprehensive assessments of patients referred for hospice services. This role involves evaluating patients medical histories coordinating care with the interdisciplinary team and facilitating a smooth admission process. The nurse will also provide education and support to patients and their families regarding hospice services.
Primary Responsibilities:
- Conduct comprehensive assessments of patients referred for hospice addressing physical emotional and spiritual needs.
- Collaborate with physicians and the interdisciplinary team to develop and adjust individualized care plans.
- Educate and support patients and families during the hospice admission process ensuring understanding and compassion.
- Complete required documentation accurately and promptly.
- Ensure all medical and administrative admission requirements are met.
- Participate in team meetings case conferences and ongoing training.
- Perform regular hospice visits including death visits and participate in the on-call rotation.
- Stay current with hospice best practices policies and regulatory standards.
- Perform other related duties as assigned.
Qualifications:
- Education & License: Active RN license required; hospice/palliative care certification (e.g. CHPN) a plus.
- Experience in hospice or palliative care settings is preferred along with a strong understanding of end-of-life care principles and best practices.
- Strong clinical assessment communication and interpersonal skills with a compassionate patient-centered approach.
- Certifications: BLS required; ACLS preferred (based on care setting).
- Technical Proficiency: Proficient with EHR systems and healthcare technologies.
- Other Requirements: Ability to work independently respond to crises and adapt to diverse social cultural and religious backgrounds. Must have reliable transportation a valid drivers license and auto liability insurance.

Compensation:$104000 - $118000 per year depending on experience.
All rates are negotiable within this range and will be discussed during the hiring process.
