About the Role

As a Support Coordinator youll walk alongside individuals as they navigate the NDIS and utilise their NDIS plans. Youll help coordinate supports build capacity and foster independenceensuring each person has the tools and connections they need to thrive.

The position is permanent Part Time 60 hours a fortnight SCHADS award Level 4.1

Key Responsibilities

  • Build strong relationships with participants and their support network
  • Build capacity of participants and their families to utilise and manage their NDIS plan
  • Coordinate services and supports across multiple providers
  • Monitor progress and NDIS funding allocations
  • Advocate for participants and promote choice and control
  • Maintain accurate documentation and reporting

What Were Looking For

  • Experience in disability mental health or community services
  • Relevant qualifications in Social Work Community Services Human Services or similar
  • Strong understanding of the NDIS legislation practice standards and guidelines
  • Excellent communication and interpersonal skills
  • Ability to work independently and manage time effectively
  • Current drivers license and reliable vehicle (if applicable)
  • NDIS Worker Screening Check and Working with Children Check

The successful applicant will have or be willing and able to obtain:

  • Working with Children Check (Blue Card) or willingness to obtain
  • Current NDIS Worker Screening check or willingness to obtain
  • Demonstrated reliability flexibility and a positive fun attitude
  • Excellent communication skills including verbal written and listening
  • Competent using technology Smart Phone and Apps
