We have an exciting opportunity for a Primary Care Recovery Coordinator who has the skills and experience to assess and engage with our service users ensuring that they access the right intervention at the right time in a way that best meets their needs.
Where: Southampton
Full Time Hours: 37.5 per week
Full Time Salary: 27861.26 - 32002.35 (pro rata for part time hours)
Contract: Fixed Term until 31st March 2026.
*Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles the salary and payments will be pro rata based on contracted hours.
Responsibilities
About the role:
- Accept referrals assess need and make appropriate recommendations for care.
- Consult with Primary Care (GPs) community mental health services mental health hospitals IAPT services and other providers of mental health and substance use support to give services users a robust package of care to support their recovery.
- Be responsible for maintaining a full case load of service users with complex needs.
- Conduct comprehensive assessments construct care packages with multi-disciplinary partners where required review care plans in a structured format and manage onward referrals.
- Conduct home visits to service users to support those with disabilities and to those with family responsibilities.
- Develop and sustain links with multi-disciplinary including statutory and non-statutory providers Criminal Justice and wrap around services.
About you:
- Excellent knowledge and understanding of drug and alcohol issues and experience of working within a related field.
- Excellent knowledge and understanding of prescription drug dependence related health and co-morbidities or ability and willingness to learn.
- Experience of working with clients with complex physical and/ or mental health needs including co-morbidity.
- Experience of working in a highly pressurised environment and the ability to use strategies to prioritise and manage work-load.
- Excellent team working and interpersonal skills maintaining a highly cooperative approach to supporting colleagues in delivering service objectives.
