Roles and Responsibilities of a Patient Care Assistant (PCA) / Home Care Aide
Personal Care Assistance
Health Support
- Observe and report changes in clients physical or emotional condition to supervising nurse or care coordinator.
- Assist with prescribed exercise or therapy programs as directed.
- Provide support for clients using assistive devices (walkers wheelchairs oxygen etc.).
- Monitor and encourage adherence to care plans and safety precautions.
Nutrition & Meal Support
- Assist with meal planning and preparation according to dietary needs.
- Encourage adequate food and fluid intake.
- Provide assistance with feeding if needed.
- Support safe use of kitchen appliances (microwave toaster air fryer etc.).
Household Support
- Perform light housekeeping to maintain a safe clean living environment including:
- Laundry bed-making and organizing personal belongings
- Washing dishes dusting and vacuuming
- Grocery shopping and errands as directed
- Assist with maintaining supplies (personal care items food and household products).
Companionship & Emotional Support
- Offer social interaction and companionship to reduce isolation.
- Engage clients in conversation activities and hobbies.
- Support client independence while providing encouragement and reassurance.
Administrative/Documentation
- Record daily care activities and observations in care logs or agency forms.
- Report promptly to supervisors regarding any incidents concerns or changes in condition.
- Follow agency policies confidentiality requirements and state/federal regulations.
