About the Aged Care Assessment Officer - Home Support role

Join our passionate Aged Care Assessment Team and make a real impact in the lives of community members. Conduct home support assessments to create tailored care plans and empower clients to stay independent while working in a supportive inclusive and community-focused environment.

This part-time role (0.7 FTE) is offered as a fixed-term contract until 26 November 2027 reporting directly to the Aged Care Assessment located in the Northern Suburbs.

Your Passion and Commitment will see you:

  • Conduct Home Support assessments via My Aged Care
  • Develop Support Plans to help clients stay independent
  • Collaborate with clients to set goals and make referrals
  • Empower clients through choice and tailored services
  • Ensure compliance with standards and regulations

To succeed in this role you will have:

  • Qualification in aged care health or related area (desirable)
  • Current Victorian Drivers Licence
  • Conduct holistic aged care assessments and tailored care plans
  • Support client choice and involvement in all care decisions
  • Contribute ideas in groups to drive system improvements
