Position Overview
We are seeking a dedicated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to support individuals in a community-based residence. This role emphasizes reliable consistent and compassionate care that aligns with our core values of dignity respect and independence.
Key Responsibilities
Health & Wellness Support
Monitor and assess individuals health needs; communicate findings to the management team.
Promote wellness through evidence-based nursing practices.
Coordinate with clinics outside physicians and hospitals; attend medical appointments as needed.
Provide care and support for individuals with G-tubes J-tubes trachs and other medical needs.
Clarify and follow through on hospital discharge plans medication orders and care plans.
Communication & Collaboration
Maintain open respectful communication with individuals supported staff family members and healthcare providers.
Participate in hospital discharges intakes and interdisciplinary team meetings alongside the Director of Nursing.
Offer education and instruction on medical diagnoses medications allergies diets and both acute and chronic conditions.
Documentation & Compliance
Maintain accurate and up-to-date medical records for all individuals supported.
Update EMAR (Electronic Medication Administration Records) and report any medication errors or concerns.
Supervise proper disposal of discontinued medications and supplies per legal requirements.
Training & Supervision
Provide training and ongoing support to direct support staff regarding medical needs care routines and emergency procedures.
Monitor staff competency and recommend further training when needed.
Support staff responsible for ordering and stocking medications.
Qualifications
RN: Associate or Bachelors degree from an accredited nursing program.
LPN: Licensed Practical Nurse degree.
Valid Rhode Island Nursing License (RN or LPN).
Experience caring for individuals with chronic acute and mental health conditions.
Proof of COVID-19 vaccination per RIDOH guidelines.
