Job Summary
Home Health Physical Therapist- Travel Position
Location: Fort Stockton TX
Job Type: Travel Contract
Duration: 13 weeks
Shift: 5x7s
Pay: $2475/week
Job Description:
We are seeking a compassionate and skilled Home Health Physical Therapist to provide rehabilitation services to patients in their homes. The PT will evaluate patient conditions develop personalized treatment plans and help patients improve mobility strength and independence. The ideal candidate is patient-focused self-motivated and comfortable working independently in a home health setting.
Key Responsibilities:
Conduct patient evaluations and develop individualized therapy plans.
Provide therapeutic exercises mobility training and functional activities.
Educate patients and caregivers on exercises safety and home modifications.
Monitor progress and adjust treatment plans as needed.
Document visits assessments and outcomes accurately in compliance with regulations.
Collaborate with physicians nurses and the interdisciplinary care team.
Qualifications:
Doctor of Physical Therapy (DPT) or Masters/Bachelors Degree in Physical Therapy
Current state Physical Therapy license
Basic Life Support (BLS) certification COVID vaccine recent TB test
Previous experience in home health or rehabilitation
Valid drivers license and reliable transportation
