Job Summary

RN- Case Manager (Permanent Role)

Location: Tallahassee FL 32301

Shift: 07:00 AM - 05:00 PM

Guaranteed Hours: (40 hours/week)

Compensation: Based on experience (Negotiable)

Essential Functions:

Perform comprehensive assessments of psychosocial and medical needs for assigned patients.
Develop individualized case management plans of care that address clinical psychosocial and discharge needs.
Coordinate and document the plan of care in the medical record ensuring communication across clinical disciplines.
Act as a leader with the interdisciplinary team to manage care through criteria-based processes for care level patient status and resource utilization.
Facilitate interdisciplinary team meetings to collaborate on implement and evaluate care plans.
Ensure that patient selection for case management is criteria-based and that all interventions are thoroughly documented.

Qualifications:

Experience:

Minimum of 3 years of recent Case Manager experience in an acute care setting (within the past year).
Will also consider candidates with 3 years of experience in any of the following units:
- Med/Surg
- Telemetry
- Neuro
- ICU
- PCU
- Emergency Department (ED)
Home health or insurance case manager experience is acceptable if the candidate has:
- A total of 3 years acute care experience and
- At least 1 year of acute care experience within the last 5 years

Education:

Associates Degree in Nursing (ADN) or Nursing Diploma Required
Bachelors Degree in Nursing (BSN) Preferred

Licensure:

Current Florida RN license or compact state RN license
- If holding a compact license must obtain active FL RN license within 90 days of hire
Advanced Practice Registered Nurse (APRN) license is acceptable if current and in compliance

Certifications (Preferred):

Case Management Certification (CCM ACM)
Nursing Certification (e.g. RN-BC)
Utilization Review Certification
