Job Summary:
This role is for a Physical Therapist providing skilled home health services to adult patients. The therapist will manage a caseload of 4-5 patients daily potentially visiting up to 6 patients per day and will be responsible for patient assessments treatment planning and documentation using provided tools. A minimum of one year of recent skilled home health physical therapy experience is required.

Responsibilities:
- Provide skilled physical therapy services to adult patients in their homes.
- Develop and implement individualized treatment plans.
- Conduct patient assessments and document findings accurately and timely.
- Manage a caseload of 4-5 patients per day with potential for up to 6 visits.
- Utilize provided documentation tools and ensure timely submission.
- Maintain timely communication with patients and other healthcare professionals.
- Adhere to all relevant regulations and standards of practice.
- Manage travel to patient homes within a defined territory (occasional maximum radius of 100 miles).
- Return provided documentation tools.
Required Skills & Certifications:
- Current Physical Therapist license in Alaska (AK).
- Minimum of 1 year of recent (within the last 3 years) skilled home health physical therapy experience (inpatient experience not accepted).
- Basic Life Support (BLS) certification.
- Proficiency in OASIS documentation.
- Reliable transportation valid drivers license and car insurance.
