Job Summary:
This Registered Nurse position requires providing skilled nursing care case management and hospice care to patients facing age-related frailty and serious illness. The role involves patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. A minimum of two years of experience is required.

Responsibilities:
- Patient assessments and monitoring.
- Medication administration and documentation.
- Treatment planning and implementation.
- Collaboration with physicians other nurses and healthcare professionals.
- Case management and coordination of care.
- Providing skilled nursing care in various settings (hospital home health hospice).
- Maintaining accurate patient records and documentation.
Required Skills & Certifications:
- Clinical assessment medication administration and treatment planning skills.
- Minimum two years of experience in hospital home health or hospice care.
- Interdisciplinary team collaboration skills.
- Excellent communication and interpersonal skills.
- Active Registered Nurse license in the state of employment.
- Bachelors degree in Nursing or related field.
- Drivers license car insurance and personal vehicle for field work.
Preferred Skills & Certifications:
- Experience in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU or Operating Room settings.
Special Considerations:
- On-call responsibilities are required.
- Floating between units is required.
- Block scheduling may be considered at the managers discretion.
- Return to office (RTO) is required.
- The facility is an acute care hospital with 150 beds.
Scheduling:
- Tuesday-Friday 11:00 AM - 9:30 PM.
- Specific shift assignments and weekend requirements are to be determined.
