Job Description
- Assess plan develop implement and evaluate programmes to promote health and wellbeing.
- Implement and evaluate individual treatment plans for patients with a known long term condition.
- Proactively identify and manage as appropriate treatment plans for patients at risk of developing a long term condition.
- Prioritise health problems and intervene appropriately to assist the patient in complex urgent or emergency situations.
- Support patients to adopt health promotion strategies that encourage patients to live healthily.
- Provide information and advice on prescribed or over the counter medication on medication regimens side effects and interactions.
- Support and manage health needs of women presenting for cervical cytology consultations.
- Recognise assess and refer patients presenting with mental health needs.
About the Candidate
- Current NMC registration with an active PIN.
- Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications.
- Long term conditions experience desirable.
- Evidence of clinical skills and experience in treatment room consultation phlebotomy ECG recording screening management of long-term conditions cervical cytology sampling vaccinations and immunisations assisting with minor surgery dressing and wound care and recording spirometry.
- Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. The successful candidate will value the dignity of our patients their relatives and carers as well as their colleagues at the practice.
- Knowledge and experience of its application in supporting long-term conditions.
- Insight and understanding of current issues in nursing and the wider NHS.

We are a large training practice based in the town of Newport Pagnell with another surgery in the Willen area.
The practice nursing team forms part of our integrated nursing team working alongside our community nursing team community matrons urgent care team and specialist nursing team.
