Summary:

Vighter is recruiting Registered Pharmacists for both outpatient and inpatient positions at the Phoenix VA Medical Center. Registered Pharmacists dispense prescription medications and provide information to patients about the drugs and their use. They also advise physicians and other healthcare workers on the selection dosage interactions and side effects of medications to treat health problems. A pharmacist in this assignment handles routine medication-related activities in accordance with local Veterans Integrated Service Network (VISN) and national policies and regulations.

These include but are not limited to: reviewing interpreting and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patients medications allergies labs and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); and providing oversight of technical staff in all aspects of medication distribution.

Key Responsibilities:

  • Perform a full range of pharmacist procedures. Reviews the prescribers medication order and patients drug regimens to assure that the medications ordered are prescribed appropriately in correct dosages and free of potential drug interactions or incompatibilities which could alter the desired therapeutic response. Counsel patients regarding appropriate use of medications.
  • Deliver comprehensive pharmaceutical services within the personnel supply and equipment capabilities of the facility.
  • Reconstitutes preparing and compounding commercial non-routine and/or special formulation parenteral admixtures by means of adhering to current USP 797/800 standards. Ensuring that preparations are unaltered therapeutically by incompatible or interacting drugs.
  • Consulting with patients providing information and answering questions regarding their drug regimen which includes advising patients of potential drug-related or health-related conditions which may develop from the use of the medication for which the patient should seek additional
  • medical care as appropriate. Review patient profiles at the time of dispensing in order to closely monitor medication therapy.
  • Ensure prescription directions for use dose medication interactions therapeutic duplications or overlaps allergy information age specific dosage and other pertinent data are appropriate for individual patients.
  • Compounds medications as required; Ensure accuracy of all pharmaceutical products prepared or processed by technical staff.
  • Supervise inventory control of all pharmaceuticals in the pharmacy especially scheduled pharmaceuticals and ensure pharmacy security.
  • Maintain appropriate records and security of all scheduled controlled substances according to applicable regulations.
  • Other duties as assigned.

Experience & Education:

  • BS or Pharm D. Degree from an accredited university or college of pharmacy.
  • 2 (two) years of experience is preferred.
  • Full current and unrestricted license to practice pharmacy in a State Territory Commonwealth of the United States (i.e. Puerto Rico) or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full and unrestricted licensure.

Qualifications:

  • Must be able to walk stand bend and/or sit for more than 8 hours per day.
  • Must be able to use an elevator or walk up and down stairs.
  • Must be able to lift up to 50 pounds and carry up to 10 feet.
  • Must be able to routinely grasp or handle objects use finger dexterity bend elbows/knees and reach above and below shoulders.
  • Must be able to read and interpret handwritten and typewritten print.
  • Must be able to communicate by voice and detect sound by ear.
  • Must be willing to work day night weekdays weekends holidays and overtime if necessary.
  • Must be able to perform duties in an indoor and/or outdoor environment.
  • Must be able to pass a criminal background investigation. Candidate must not have been convicted of pled guilty to pled nolo contender to or received probation prior to judgement for any crime other than a minor traffic violation the record of which has not been expunged.
  • Must undergo Urine Drug Screening (UDS).

Duty Hours / On-call Requirements

  • Pharmacy activities are open 24/7/365 to include days nights weekends and holidays. Acceptable candidates must be willing to work as needed.
