About the role
The Restorative Care Partner is responsible for theclinical coordination and delivery of restorative and rehabilitative care servicesin accordance with a clients support Restorative Care Partner ensures optimal health outcomes for clients while promoting independence and functional recovery and ensures culturally responsiveholistic and inclusive careis provided.
This full-time permanent position is based at our Bella Vista NSW office with hybrid working arrangements available.
The key aspects of this role will include:
- Community & stakeholder engagement
- Assessment & restorative care coordination
- Ongoing support advice & guidance
- Quality care processes & risk management
About You
To be successful in this role you will have:
- Current registration as a Registered Nurse or Allied Health professional
- Specialist certifications / qualifications in relevant disciplines are highly regarded
- Extensive experience inclinical coordination case management or restorative care within aged care disability or community services.
- Strong understanding ofreablement functional rehabilitation and person-centred care models.
- Proficiency inrisk management compliance and quality assurance processes.
- High-level communication problem-solving and decision-making abilities.
- Competence in usingclinical documentation and care management systems.
- Exceptional customer service interpersonal and communication (listening written and verbal) and liaison skills with the ability to deliver dynamic person-centred service within the community and in clients homes.
- Proven experience working in a fast-moving human services environment managing a high workload providing high quality advice and support for a range of client needs
- A sound understanding of the needs and experiences of people who receive community care services and of the community care system in relation to the program
- Competence and confidence in building productive networks and relationship development
- An ability to take initiative problem solve and make decisions
- Experience working to operational key performance indicators (KPIs) and achieving business development targets in a performance-driven environment
- Experience managing client budgets being responsive and accountable for time sensitive tasks
- Drivers licence & own car
