Job Summary

Description

We are looking for a compassionate and dedicated Social Worker to join the team at Center on Aging and Health. As a Social Worker you will play a crucial role in providing support and assistance to our residents. You will be involved in a variety of projects aimed at improving the social welfare of the elderly population.

As a Social Worker at Center on Aging and Health you will have the opportunity to work closely with residents their families and other healthcare professionals to address social emotional and financial challenges. Your role will involve conducting assessments developing care plans providing counseling and connecting clients with community resources. This position requires a strong sense of empathy excellent communication skills and the ability to collaborate effectively with multidisciplinary teams.

Responsibilities:

Conduct assessments to identify the social and emotional needs of our residents.
Develop individualized care plans to address resident specific challenges and goals.
Provide counseling and emotional support to residents and their families.
Connect clients with community resources such as housing assistance transportation services and support groups.
Collaborate with healthcare professionals including doctors nurses and therapists to coordinate care.
Document client interactions progress and treatment plans accurately and timely.
Participate in care conferences and team meetings to discuss client progress and adjust care plans as needed.

Requirements:

Excellent communication and interpersonal skills.
Ability to work effectively in a fast-paced and dynamic environment.
Empathy and compassion for the elderly population.
Experience working with seniors or in a healthcare setting is a plus.
Knowledge of community resources and support services for seniors.
