The Woodlands Hospital a primary and secondary care hospital serving North Harris and Montgomery counties. Clinical services include cardiovascular services diagnostic imaging womens services (digital mammography and bone density studies) neurosciences pediatric care (Newborn and Level II nurseries) and surgery pathology and pulmonary services and sleep disorders. Clinical affiliations include The University of Texas MD Anderson Cancer Centers Radiation Treatment Center and Texas Childrens Hospital.
As a Care Coordinator RN with CHI health you will coordinate the care and services of patients across the continuum of illness; promote effective utilization; monitor health care resources; and assume a leadership role with the interdisciplinary team to achieve optimal outcomes through the tools of case management. The case manager will perform a wide variety of high-level patient care activities to include admission assessment screen for appropriateness and development of a discharge plan. Timely and appropriate discharges will be accomplished by evaluating the patients post-discharge needs and the provision of services necessary to meet those needs. The case manager must be knowledgeable in community resources and alternate care facilities.

MajorExperience/Education:
Minimum two (2) years of acute hospital clinical
experience with ADN or a Masters degree in Case Management
or Masters Degree Nursing TX RN License
