About The Role:

The Mobile Health Services Coordinator oversees the daily activities of Community Health Workers (CHWs) and ensures outreach education and coordination of services that promote improved health outcomes. This role focuses on increasing awareness of health services coordinating mobile health services and community clinics and delivering health and wellness education. The Mobile Health Unit is an essential extension to provide equitable healthcare access to Latine immigrant and historically underserved communities to address critical healthcare needs and reduce barriers to care.

Principle Duties & Responsibilities:

Supervision & Mobile Health Services Coordination

  • Provides direct supervision coaching training and support to Community Health Workers
  • Monitors performance including annual reviews goal setting and professional development
  • Guides case management and implements best practices.
  • Coordinates mobile health service operations including scheduling staffing community-based health clinics screenings and wellness events in collaboration with Community Health Workers
  • Maintain effective coordination and communication with assigned funder liaison and related staff.
  • Attend scheduled program meetings with funders school districts and community.
  • Actively participate in leadership department and all-staff meetings.

Contract Management

  • Manage program related contracts including:
  • Mobile Health Services
  • OHP Health Systems Navigation
  • Other contracts and programs as assigned.
  • Coordinate outreach activities to connect with OHP-eligible individuals and communities with a focus on increasing access to primary care preventive services and wellness programs.
  • Build and maintain partnerships with local organizations clinics and community leaders to expand program reach.
  • Develop culturally responsive outreach strategies tailored to diverse communities.
  • Oversee the development and delivery of community-based health education programs on topics such as chronic disease prevention nutrition mental health and OHP benefits.
  • Ensure educational materials are accurate culturally appropriate and accessible to diverse populations.
  • Monitor budgets and resources related to outreach mobile health services and educational activities.
  • Ensure EPHCs policies and procedures are communicated and followed.
  • Perform other duties as assigned by supervisor.

Data Management

  • Maintain accurate records data tracking and reporting in compliance with funding and program requirements.
  • Ensure all participant intakes assessments service plans progress notes financial verification forms and other required documents in case management adhere to State Federal and EPHC standards/guidelines of service-delivery and Code of Ethics.
  • Prepare all required reports as required by funders and EPHC

Qualifications & Requirements:

  • Bachelors Degree in Public Health Community Health Social Work or related field AND a minimum of 1 year of experience or the equivalent of 5-8 years of qualifying training and/or experience
  • Supervisory experience preferred.
  • Community Health Worker Certification preferred.
  • Bilingual in Spanish and English with strong bicultural competency and the ability to communicate clearly and effectively in both languages (verbally and in writing).
  • Ability to lead group classes and engage with diverse community members.
  • Strong verbal and active listening skills to lead meetings engage with community members and facilitate training sessions.
  • Excellent organizational skills with the ability to prioritize tasks make proactive decisions and follow through.
  • Attention to detail in managing program records data entry and report preparation.
  • Ability to work independently and adapt to changing priorities while collaborating effectively with a team.
  • Strong proficiency in Microsoft Office programs

Other Requirements:

  • Must successfully pass all required background checks.
  • Flexibility to work varied hours including evenings and weekends.
  • Willingness and ability to travel occasionally for training workshops or organizational needs.
  • Occasional work outside standard office environments including on-site visits community outreach home visits etc. at multiple locations.
  • Reliable transportation is required for work-related travel. If driving employees must have a valid drivers license access to a personal vehicle and automobile insurance that meets the agencys minimum coverage requirements (100/300/100).
  • Ability to sit stand and walk for extended periods during meetings food distribution and site visits.
  • Ability to lift and carry materials up to 40 pounds.

Compensation: $60320.00-$62400.00 (annually). Salary commensurate with skills and experience.

Location & Typical Working Conditions:

Community (60% of time)

  • Coordinate mobile health services conduct outreach lead community tabling events coordinate workshops attend in-person training community partner meetings etc.

In office (20% of time)

  • In office work is located at EPHCs office in Gresham OR and is required for staff meetings department meetings events and other additional duties requiring in office work.

Remote from home 20% of time)

  • This position may allow remote work from home for administrative tasks such as data entry expense report submission funder report writing and partner and work-related virtual meetings. Note: Remote work is dependent on performance and may be revoked at supervisors discretion
