Job Description
Position: Licensed Practical/Vocational Nurse(LPN/LVN) - Long Term Care (LTC)
Start Date: 9/23/2025
Duration: 13 weeks
Location: Bad Axe MI 48413
Shift Type/Shift Times/Hours/Day: Nights 12
Shift Notes: 6p-6a
Weekend Requirements: Every other
On-Call Requirements: Yes
COVID-19/FLU Requirement: na
Traveler Policy: Candidates Permanent Address (Facility will not consider local candidates -35-mile radius restriction)
OT starts after 48 hours
OT/Holiday 1.5 x bill rate
- Provides supportive services to nurse(s) and other staff as needed and performs duties as assigned.
- Assists resident with or performs activities of daily living for resident in accordance with care plans and established policies and procedures.
- Assists resident with lifting turning moving positioning and transporting into and out of beds chairs bathtubs wheelchairs lifts etc.
- Coordinates dining room services at assigned mealtimes including set-up and clean-up meal tray delivery feeding assistance and documentation of meal intake.
- Delivers nutritional supplements to residents at assigned times and assists as necessary to ensure intake. Documents intake accordingly.
- Assists with weighing residents according to facility policy and records weight in designated locations. Reports weight changes to the nurse and supervisor.
- Assists with tracking the condition of the residents skin. Reports any presence of pressure areas skin breakdown or skin tears to the nurse and supervisor.
- Assists nursing staff in carrying out toileting program activities.
- Completes flow sheets daily to indicate that the specified task was done. Documents the progress of each resident as directed by the supervisor.
- Keeps nurse and supervisor informed of factors that interfere with being able to perform the work as assigned (i.e. resident refusal reassigned to nursing unit).
- Reports changes in a residents condition to the residents nurse and supervisor.
- 1 year of experience in LTC- Required
- BLS - Required
- MUST HAVE TRAVEL EXPERIENCE
- Must have a successful work history showing staying at previous work longer than 13 weeks
- No disciplinary action is in effect against certification
