Job Description
- Dress code: Business professional
Responsible for the transition of care/discharge planning for both inpatients and our skilled rehab patients.
- Triages coordinates and facilitates the clinical management for an assigned population.
- Develops monitors and reinforces care standards and protocols designed to enhance quality and promote cost-effective utilization of healthcare resources.
- Manages the care of assigned patients from intake through discharge and follow up.
- Coordinates the delivery of services to patients and families.
- Manages the financial resources of the patient by acting as the liaison to the payer source and assuring cost-effectiveness of the treatment provided by the team.
- Facilitates and coordinates team conference reporting and the discharge planning process.
- Serves as the patient and family advocate to coordinate and direct effective communication regarding the patient s care.
- Performs post-discharge follow-up.

Local Travelers - Those who reside within 50 miles of the facility - are not eligible for this need.
This is a small community without public transport. Traveler candidate must have their own vehicle. This is a firm requirement.
8hrs non-billable orientation
Holiday and OT multipliers: 1.25%

- Minimum 3 years clinical experience required
- Recent acute care hospital experience required
- BSN preferred
- Previous travel preferred
- Utilization review experience a plus
- Traveler must have their own vehicle for this need
