The RN will perform patient assessments administer medications develop treatment plans and collaborate with an interdisciplinary team. Strong communication and collaboration skills are essential. Responsibilities:
- Performing patient assessments and developing treatment plans.
- Administering medications and treatments.
- Collaborating with physicians other nurses and other healthcare professionals.
- Providing skilled nursing care case management and hospice care.
- Maintaining accurate patient records.
- Educating patients and their families.
- Participating in interdisciplinary team meetings.

Required Skills & Certifications:
- Clinical assessment medication administration and treatment planning skills.
- Two years of minimum experience in hospital home health or hospice care.
- Interdisciplinary team collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Active Registered Nurse license in the state of employment.
- Bachelors degree in Nursing or related field.
- Drivers license car insurance and automobile for field work.
- Two years of minimum experience in hospital home health or hospice care.

- Experience in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU or Operating Room settings.

- The role may involve working in a fast-paced environment with patients facing advancing age frailty and serious illness. This may include hospice care and case management responsibilities.
