DescriptionThe Structural Heart Program Coordinator is responsible for overseeing all aspects of the Structural Heart program ensuring high-quality patient care regulatory compliance and optimal program performance. This role serves as the central point of coordination between patients physicians advanced practice providers and multidisciplinary team members. The coordinator facilitates patient evaluation procedural planning data management and quality improvement initiatives.
ResponsibilitiesClinical Coordination:
- Coordinate the entire Structural Heart patient journey including referral intake screening diagnostic testing Heart Team review scheduling and post-procedural follow-up.
- Serve as the primary liaison between patients and the Structural Heart team to ensure timely communication and care coordination.
- Participate in patient and family education setting expectations and answering questions related to the procedure and recovery process.
Operational Support:
- Schedule and facilitate Heart Team meetings and case conferences.
- Collaborate with cardiovascular imaging interventional cardiology cardiac surgery and other departments to streamline workflow and ensure procedural readiness.
- Maintain clinical pathways and standard operating procedures for the Structural Heart program.
Data & Quality Management:
- Ensure timely and accurate data entry into national registries.
- Monitor program metrics and key performance indicators (KPIs) including outcomes volumes and compliance benchmarks.
- Collaborate with quality and compliance departments to support audits accreditation and continuous quality improvement initiatives.
Regulatory Compliance:
- Ensure compliance with CMS National Coverage Determination (NCD) requirements and institutional policies.
- Maintain documentation and support processes necessary for program accreditation or certification.
Education & Outreach:
- Participate in community outreach and physician education to promote awareness and appropriate referrals to the Structural Heart program.

QualificationsMinimum Required Experience Type:3 years clinical experience
Minimum Required Education:Bachelors Degree required
