Key Responsibilities:
- Deliver direct patient care through medication therapy management and clinical interventions
- Collaborate with physicians nurses and interdisciplinary teams to ensure safe and effective drug utilization
- Review verify and interpret medication orders for accuracy and appropriateness
- Monitor patient responses and adjust therapies based on clinical data and outcomes
- Educate patients and healthcare staff on drug interactions side effects and proper usage
- Participate in formulary management stewardship programs and quality improvement initiatives
- Maintain compliance with state and federal pharmacy regulations and institutional policies
Qualifications:
- B.S or Doctor of Pharmacy (Pharm.D.) from an accredited institution
- Active pharmacist license in the state of practice
- Completion of PGY1 residency (PGY2 preferred for specialty roles)
- Board certification (e.g. BCPS BCACP) strongly preferred
- Proficiency with EHR systems and clinical decision support tools
- Strong communication analytical and documentation skills
