MaineGeneral Health is seeking a compassionate and skilled Registered Nurse (RN) to join our Community Care this role youll partner with patients families healthcare providers and community resources to improve the health and quality of life for those with the highest risk highest cost and highest utilization of health care services.

Location: Ballard Center Augusta
Schedule: Part-time (32 hours/week)
Shift: Days M-F
The Role:
As a Care Manager on the Community Care Team youll work with individuals who face complex medical behavioral and social challenges. Many of these patients have frequent hospital or ER visits and your mission will be to help them stabilize stay healthier and reduce avoidable utilization.
What Youll Do:
- Interviews and assesses patients and/or patients family caregivers and/or legal representatives.
- Determines prioritizes provides and/or arranges for needed internal and external services/interventions. Consults with multidisciplinary teams regularly to evaluate patients status and appropriateness of medical care including admission length of stay transfer and discharge.
- Participates in discharge planning including coordinating patient transfers to other facilities and coordinating community resources. Provides discharge education and resource referrals to patients.
- Performs chart review to identify actual or potential issues with service delivery patient outcomes and satisfaction compliance cost and reimbursement.
What You Bring:
- Active Registered Nurse license in the State of Maine (required)
- Excellent communication teamwork and advocacy skills
- Strong critical thinking and problem-solving abilities
- Ability to build trust and form supportive relationships with patients from diverse backgrounds
- State of Maine drivers license with an acceptable driving record
