- Barossa Hills Fleurieu Local Health Network Community and Allied Health
- Position based in Mount Barker
- Salary: $87416 -$110497 p.a (pro rata) superannuation leave loading salary sacrifice benefits
- Ongoing Part Time working 7.6 hours per fortnight
WHAT ARE WE OFFERING
We are looking for a skilled and compassionate Geriatric Clinical Nurse to join our dedicated team within the Barossa Hills Fleurieu Local Health this role you will provide expert clinical nursing care and support the development and delivery of effective Geriatric Outpatient services. You will play a vital role in coordinating holistic person-centred care for older adults across multidisciplinary teams helping to improve health outcomes for vulnerable populations.

WHO ARE WE LOOKING FOR
- Do you have experience providing clinical nursing care to older adults with a strong background in geriatrics or aged care
- Are you skilled in coordinating care and managing referrals within a multidisciplinary team
- Can you demonstrate effective communication and interpersonal skills to work with people from diverse cultural and linguistic backgrounds
- Are you motivated to engage with Aboriginal consumers in a culturally respectful way and eager to undertake further cultural safety training

HOW WILL YOU CONTRIBUTE
- Provide proficient person-centred clinical nursing care and individual case management to older adults in a defined clinical area.
- Support the coordination triage and prioritisation of patients referred to geriatric outpatient clinics.
- Collaborate effectively with multidisciplinary teams and other aged care services to ensure holistic care and service integration.
- Lead and support clinical education mentoring less experienced staff and students and contribute to continuous quality improvement initiatives.
