Job Description

Department: Community Health Services

Job Status: Full Time

FLSA Status: Exempt

Reports To: Community Health Services Director

Grade/Level: Salaried

Amount of Travel Required: 10%

Job Type: Regular

Positions Supervised: Service Coordinators Home Visitors and Public Health Nurses

Work Schedule: 8 am - 4 pm M-F with one-hour unpaid lunch. Work hours days and times will vary based on work requirements and/or emergencies.


POSITION SUMMARY:

Planning oversight and review of one or more of the Help Me Grow Home Visiting Early Intervention Nurse Family Partnership Family Connects Communicable Disease HIV and Complex Medical Help programs.


ESSENTIAL FUNCTIONS:

  • Timely regular and consistent attendance.
  • Project a positive and professional image. Provide respectful customer service to external and internal clients.
  • Preparation of proposals and grant applications and performance of responsible professional and administrative work in researching identifying developing and responding to public and private grant opportunities.
  • Provide program information in a positive manner within the agency and the public either through formal training or informal communication; distribute pamphlets brochures and booklets to the public; and utilize various types of communication to convey program information.
  • Assist in budget grant and contract preparation; monitor budget and make recommendations during the budget process. Oversee program budget to assure compliance with state audit requirements. Provide assistance with billing approval.
  • Maintain program compliance and ensure appropriate service delivery.
  • Review the quality and quantity of service coordinators home visitors and/ or Public Health Nurses activities by accompanying them as required by the program including approved parenting education curriculum and screenings as indicated in the family plan. Ensure that service coordinators home visitors and/ or Public Health Nurses work is completed as prescribed by the program and health district policies and procedures. Review records and reports to ensure timelines are met.
  • Supervise & manage personnel including implementing a performance improvement plan as needed and according to agency policy.
  • Utilize appropriate methods for interacting effectively and professionally with people of all ages and from diverse cultural socioeconomic educational racial and ethnic backgrounds sexual orientations lifestyles and physical abilities.
  • Use quality improvement (QI) and performance management (PM) processes and/or techniques to improve program effectiveness and services including but not limited to creating implementing and evaluating performance standards and identifying implementing and assessing QI processes.
  • Responsibilities include other duties as assigned including but not limited to Accreditation requirements and responding to emergency events.


PREFERRED QUALIFICATIONS:

Education:

Bachelors Degree (four-year college or technical school) Required Field of Study: Early Childhood and/or a field related to working with pregnant women infants/toddlers individuals/families with disabilities; Nursing; Nutrition/ Dietetics.


Masters Degree Preferred Field of Study: In Early Childhood and/or a field related to working with pregnant women infants/families with disabilities; Nursing; Healthcare Management/ Administration; Public Health.

Experience:

Bachelors degree with three (3) years experience with one year of supervisory experience required.

3 plus years of relevant experience serving children and families.


Masters Degree may be considered in lieu of experience and supervisory experience.


Computer Skills:

Microsoft Office required.


License & Certifications:

Registered Nurse

Must obtain/ retain one or multiple of the following:

  • Family Connects Supervisor Credential
  • Nurse Family Partnership Supervisor Credential

Must possess and maintain a valid drivers license.


Other Requirements:

National Incident Management Systems (NIMS)/Incident Command System (ICS) 100200 and 700.a certification must be obtained within six months of hire. NIMS/ICS 200 400 and 800.B to be obtained within 12 months of hire.

Professional office environment. Work related travel may subject employee to conditions that
