Job Description

Clinical Manager
Reports to: Executive Director of Home Health Classification: Full-time; exempt
Setting: Hybrid (LEL office home office community)
Summary: The Clinical Manager for Home Health is responsible for the management and oversight of clinical services including nursing care therapy and other health services delivered to patients in their homes. This position ensures compliance with healthcare regulations maintains high-quality care standards and manages the home health care team.
Key Responsibilities:
  • Leadership and Staff Management
    • Supervise and provide direction to nurses therapists and other healthcare professionals within the home health team.
    • Conduct regular performance evaluations and provide training mentorship and professional development opportunities to staff.
    • Hire onboard and assist with scheduling of clinical staff.
  • Patient Care
    • Complete supervisory visits and 485 POC/OASIS assessments.
    • Implement/receive orders from physicians for patients.
    • Ensure that all patients receive the appropriate care as outlined in their individualized care plans.
    • Monitor patient progress and make recommendations for adjustments to care plans as necessary.
    • Ensure that all clinical documentation is accurate timely and compliant with agency policies and regulations.
  • Compliance and Regulatory Adherence
    • Ensure the home health agency is in compliance with federal state and local healthcare regulations including licensing accreditation and certification standards (e.g. Medicare and Medicaid regulations).
    • Conduct internal audits and quality checks to ensure care standards are met and that proper protocols are followed.
    • Address and resolve any patient or regulatory concerns and handle patient complaints or issues promptly.
  • Care Coordination and Collaboration
    • Collaborate with physicians social workers and other members of the interdisciplinary team to develop implement and review patient care plans.
    • Coordinate patient care with family members and caregivers to ensure they understand care needs instructions and discharge plans.
  • Quality Assurance and Improvement:
    • Oversee clinical quality improvement programs and initiatives aimed at enhancing patient outcomes and satisfaction.
    • Analyze clinical outcomes identify areas for improvement and implement corrective actions.
  • Other duties as requested
  • Bachelors Degree in Nursing from an accredited institution; Masters Degree Nursing or health-related field Preferred.
  • Current State Registered Nurse License with no restrictions.
  • Has a minimum of five (5) years experience supervising licensed staff and providing clinical supervision.
  • Demonstrates initiative in managing issues and problems.
  • Strong interpersonal and relationship building skills.
  • Customer service oriented.
  • Satisfies all requirements under applicable law.
  • A positive and pleasant attitude.
  • Ability to work independently and meet deadlines.
  • Strong organizational skills and close attention to detail.
  • Strong problem-solving skills.
  • Excellent written and oral communication skills.
  • Dependable vehicle and valid drivers license.

While performing the duties of this job the employee is regularly exposed to an office environment in which the noise level is usually low to moderate. The employee is occasionally exposed to outside weather conditions as well as Individual residences in which the noise levels are usually moderate.
