Job Summary:
This Registered Nurse (RN) position requires providing skilled nursing care case management and hospice care to patients with advancing age frailty and serious illness. The RN will perform patient assessments administer medications develop treatment plans and collaborate with an interdisciplinary team. Strong communication and collaboration skills are essential. This role involves on-call responsibilities and occasional weekend coverage.

Responsibilities:
- Patient assessments and monitoring
- Medication administration
- Treatment planning and implementation
- Collaboration with interdisciplinary team (physicians other nurses etc.)
- Case management
- Hospice care (as applicable)
- Charting in Epic system
- Responding to on-call requests (1-2 nights/week)
- Occasional weekend coverage
Required Skills & Certifications:
- Active Registered Nurse (RN) license in the state of employment
- Bachelors degree in Nursing or related field
- Minimum two years of experience in hospital home health or hospice care
- Clinical assessment skills
- Medication administration skills
- Treatment planning skills
- Interdisciplinary team collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Drivers license car insurance and personal vehicle for field work (if applicable)
Preferred Skills & Certifications:
- Experience in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU or Operating Room settings.
Special Considerations:
- On-call responsibilities: 1-2 nights per week
- Occasional weekend coverage required
- Block scheduling is used
- Patient ratios: 1:1
- 48-hour work periods are not approved
- Work environment: Short-term acute care/Level II Trauma hospital; 209 beds; teaching hospital.
