Job Description
Come join our growing team! We are an established outpatient orthopedic facility and we treat a wide variety of injuries to include a diverse orthopedic population.
Our commitment is to deliver high quality one-on-one care to our patients. We are currently looking for a dedicated and motivated individual with an enthusiastic attitude to join our team as a Physical Therapist for our clinic in Farmingdale Maine. If you enjoy caring for patients from diagnosis through full recovery this is the opportunity for you!
- Assess patients to develop and initiate treatment plans based on the evaluation results
- Review the physicians referral (prescription) and the patients condition and medical records to determine the physical therapy treatment required
- Provide direct patient care by established protocols and patient care guidelines
- Document findings progress and instructions to patients and caregivers
- Responsible for patient care performed by assistants and rehabilitation technicians
- Complete a written plan of care develop goals based on evaluation findings and develop a comprehensive program to attain goals
- Implement physical therapy treatment program and provide essential physical therapy treatments
- Communicate effectively with patients/caregivers and professional colleagues including physicians and insurance companies
- Ensure timely documentation including daily notes weekly workers compensation reports progress reports initial evaluations discharge summaries and incident reports
Qualifications :
- Graduate from a CAPTE accredited Physical Therapy Program.
- One or more year(s) of outpatient orthopedic experience within a private clinic or hospital preferred
- Current state of Maine license CPR certification
- Outgoing and energetic personality.
- Exercise and Manual based skills and Sports Rehab
