This position provides direct patient care to hospice and home health patients in their home or place of residence. The CNA assists in providing a clean and safe environment. They work cooperatively with patients families and caregivers and share observations and changes with the RN care manager. They work under the supervision of a Registered Nurse.
Minimum Qualifications:
- High School Diploma or equivalent
- Virginia State Approved CNA program Certification
- Educational requirements per hospital certifications (Example: Stroke Orthopedic/Spine)
- Current Virginia State CNA License
- Basic Life Support - BLS
Preferred Qualifications:
- Previous home health and hospice experience
Skills:
- Mandatory Competencies required by all Augusta Health Employees annually
- Transfer Training
- Must be able to read write and verbally report clinical information to patients representatives and caregivers as well as other hospice team members
- Demonstrates compassion for patients at end of life
- Ability to work in patients homes in various conditions
- Ability to work a flexible schedule
- Ability to travel to and from patient homes in various weather conditions
- 12 hours of additional in-service per year
