Job Summary
Joinathriving and growing home health organization as we expand our services intohospice. As we begin ourhospiceexpansion we arelooking for a compassionate Chaplain to support our patients and their families during challenging times.
Employment Type:This position will grow as our hospiceserviceline grows. Initially this role is envisioned as PRN or part-time with the potential to transition to full-time in the near future.
Key Responsibilities:
- Provide spiritual support and counseling to patients and their families.
- Conduct spiritual assessments of patients and implement appropriate care plans.
- Coordinate with the interdisciplinary team to ensure holistic care.
- Facilitate religious or spiritual rituals as requested by patients.
- Offer crisis intervention and bereavement support.
- Participate in the development and delivery of patient care plans.
- Ensure compliance with all federal state and local regulations regarding spiritual care.
- Participate in biweekly interdisciplinary team meetings.
Required Qualifications:
- Endorsement by a recognized religious/spiritual body.
- Experience in hospice or healthcare chaplaincy.
- Excellent interpersonal counseling and communication skills.
- Understanding of and sensitivity to a diverse patient population and their varied belief systems.
- Ability toworkcollaboratively in an interdisciplinary team.
