The Behavioral Health Provider (BHP)- Position provides direct clinical services to pediatric patients (birth-18) patients. Services provided include brief assessment and evidenced based psychotherapy interventions for patients living with medical mental health and substance use disorders. This includes performing advance comprehensive assessment and analysis of complex psycho-social factors and coordination for patients receiving medication supported recovery. The position also involves support care givers and other individuals supporting patients through care collaboration skills training and providing resources and care coordination with community entities supporting the patient. As a member of the clinic medical team the BHP provides a vital consultation role to team members. The BHP regularly advises providers and medical team members on the full-spectrum of behavioral health and substance use conditions/diagnoses and appropriate/available behavioral health and substance use treatment options. The position also requires mental health pediatric experience as well as being licensed or working towards hours for a LCSWLPC and PsyD. The position is located in the Multnomah County Medical Clinics which is a covered Federally Qualified Health Center (FQHC) under the Bureau of Primary Health Care and the Centers for Medicare and Medicaid Services of the United States Department of Health and Human Services. This position will provide support at the North East Health Center East County Health Center and Mid County Heath Center as well as provide as needed services to the North Portland South East and Fern Hill Health Centers.


As the Behavioral Health Provider other duties will include:

Integrated Behavioral Health Care

  • Provide bio-psycho-social assessment and evidence-based behavioral health interventions directly to patients 0-18 years of age with medical conditions.

  • Applies knowledge and experience in the relationship between medical and behavioral aspects of health and disease while engaging in consultation with patients and team members.

  • Participates in group medical appointments (shared medical appointments and drop-in group medical appointments) as requested by team members. Provides for unscheduled consultative appointments in a timely manner for patients when referred by another provider of the primary care team.


Mental Health and Health Compromising Behavior Care

  • Provide behavioral health assessment diagnosis and brief evidence-based behavioral interventions usually lasting from 1-6 visits for patients with behavioral health concerns.

  • Provide continuity care to patients seen.

  • Care planning may include talking or meeting with family or support members or school staff if patient in school.

  • Implementation of suicide risk assessment pediatric cognitive assessment and developmental assessment as well as assessment related to attention deficient social development substance use disorder behavioral issues and trauma.


Consultation

  • Provide expert clinical consultation and education to the Primary Care Provider teams.

  • Provide timely and succinct feedback to other primary care providers regarding consultation findings and recommendations.

  • Formulates diagnostic and treatment recommendations and presents findings to treatment teams.

  • Teach team members about behavioral health care topics and clinical issues as needed and.

  • Provides timely and succinct feedback to other Primary Care Providers regarding peer support integration and recommendations related to whole person care.


Case Management

  • Provide case management services to patients receiving behavioral health services.

  • Directly support patients to establish care with community behavioral health partners for more long term support.

  • Program development of the behavior health model in ICS as well as providing guidance and support regarding how to most effectively the care team can support behavioral health

Documentation and Quality Improvement

  • Documents all work with and on behalf of patients in the electronic health record including behavioral health visits telephone encounters and other work. Completes documentation within stated timelines.

  • Participates in quality improvement activities that support the Integrated Behavioral Health team utilizes relevant IBH data in decision making and planning.

Family/Community Education

  • Provides ongoing education to patients and patient family support verbally and/or on a face-to-face basis through educational patient activities with the goal of integration and collaboration with patient personal support when appropriate.

  • Provides orientation services for newly enrolled patients.

  • Teaches Primary Care team members about behavioral health care topics and clinical issues related to peer support.

  • Read current journals and publications to keep current on consultation and care trends to include culturally based care community interactions and connections etc.

Required Minimum Qualifications/Transferable Skills*:

Any combination of experience and training that would likely provide the knowledge and abilities is qualifying. A typical way to obtain the knowledge and abilities would be as noted below. All non-traditional education training and experience will be evaluated.

Education/Training:

  • Training and experience for this journey level professional classifications are a masters degree and one (1) to four (4) years of experience that demonstrates the ability to perform the duties of the position.

  • Depending upon assignment candidates may be required to pass a criminal background check have a valid driver license and/or additional training licenses or certificates.

Experience/Certification:

  • Preferred: Licensed Clinical Social Worker Clinical Psychologist or

  • Licensed Professional Counselor and Therapist

  • Required: Clinical Social Work associate

  • Licensed: Professional Counselor - Intern

  • psychology doctoral intern
