Qualifications
- MD/DO
- Unrestricted State Medical License
- Unrestricted DEA License or ability to obtain
- Fellowship inFemale Pelvic and Reconstructive Surgery
- Female Pelvic and Reconstructive SurgeryBC/BE (ABOG AOBOG)
- Board certification required within 5 years of eligibility
- BLS
