Job Description

Responsibilities may include:
- Ensure treatment plans are up to date for each client as appropriate.
- Coordination of the individual plans of individuals served as applicable.
- Provide assessment services effectively and efficiently.
- Complete client screenings timely as assigned.
- Complete comprehensive and succinct progress notes for each client.
- Ensure that the person served is oriented to his or her services.
- Sharing information on how to access community resources relevant to client needs.
- Communicating information regarding progress of the person served to the appropriate persons.
- Facilitating the transition process including arrangements for follow-up services.
- Involving the family or legal guardian when applicable or permitted.
- Provide trainings as requested by manager for internal and external customers as well as other co-workers.
- Obtain clinical supervision from collaborating Physician as necessary.
- Identifying and addressing gaps in service provision.
- Participate in management team meetings and other meetings as assigned.
Requirements for this position include:
- Graduate of an accredited Nurse Practitioner program.
- PMHNP license required.
- Experience working as a PMHNP is required.
- Supervisory experience may be preferred.
- Outpatient and tele-psychiatry experience is preferred.
- Willingness to work with a variety of ages including children/adolescents.
- Willingness to travel throughout assigned service area.
