Your Role with the Company:
- Full-time position Flexible schedule and work/life balance
- 60-90 min visits specializing in peri menopause GI regenerative and functional medicine
- Provider will see 3-5 patients/day home visits some clinic visits and telehealth
- Nurse Practitioner will build up to carry a patient panel of 225 patients
Background Profile:
- FNP / Family Nurse Practitioner APRN Adult Gerontology Nurse Practitioner
- License and Independent practice authority in Illinois
- 2 years of Hormone experience required functional medicine experience desired
