Job Summary:
This Registered Nurse (RN) position requires providing skilled nursing care case management and hospice care to patients facing age-related frailty and serious illness. The RN will perform patient assessments administer medications develop treatment plans and collaborate with the interdisciplinary team. Strong communication and collaboration skills are essential.

Responsibilities:
- Patient assessments and monitoring.
- Medication administration and documentation.
- Treatment planning and implementation.
- Collaboration with physicians other nurses and healthcare professionals.
- Case management and coordination of care.
- Providing skilled nursing care in various settings (potentially including Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room).
- Hospice care (if applicable).
Required Skills & Certifications:
- Clinical assessment medication administration and treatment planning skills.
- Active Registered Nurse (RN) license in the state of employment.
- Bachelors degree in Nursing or related field.
- Minimum two years of experience in hospital home health or hospice care.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Drivers license car insurance and personal vehicle for field work (if applicable).
Preferred Skills & Certifications:
- Experience in multiple specialties (Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room).
