Responsibilities:
- Assess patients needs and develop individualized care plans.
- Coordinate and monitor patient care throughout the healthcare continuum.

- Educate patients and families on healthcare options resources and self-care techniques.
- Conduct regular assessments to evaluate the effectiveness of care plans and make necessary adjustments.

Requirements:
- Associate degree in Nursing (RN) from an accredited institution.
- Active RN license in the state of practice.
- Prior experience in case management or care coordination preferred.
- Strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced healthcare environment.
- Knowledge of healthcare laws regulations and best practices.
- Excellent organizational and time management skills.
- Critical thinking and problem-solving abilities.
