We are currently seeking a dedicated Case Manager RN to join Big Sur Health Center a leading healthcare facility in Big Sur. As a Case Manager RN you will play a crucial role in coordinating patient care ensuring optimal outcomes and promoting patient well-being. You will be involved in a variety of projects aimed at improving healthcare delivery and patient satisfaction making this role essential to our organizations success in providing high-quality care.

As a Case Manager RN at Big Sur Health Center you will have the opportunity to work on interdisciplinary teams develop care plans and collaborate with healthcare providers to ensure seamless patient care transitions. Your role will involve optimizing resources advocating for patients and implementing evidence-based practices to enhance patient outcomes and satisfaction. Responsibilities:

  • Assess patients needs and develop individualized care plans.
  • Coordinate and monitor patient care throughout the healthcare continuum.
  • Collaborate with healthcare providers to ensure effective communication and care coordination.
  • Advocate for patients to ensure their needs are met and rights are respected.
  • Educate patients and families on healthcare options resources and self-care techniques.
  • Conduct regular assessments to evaluate the effectiveness of care plans and make necessary adjustments.
  • Ensure compliance with regulatory requirements and standards of care.
  • Participate in quality improvement initiatives to enhance patient care processes.

Requirements:

  • Associate degree in Nursing (RN) from an accredited institution.
  • Active RN license in the state of practice.
  • Prior experience in case management or care coordination preferred.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a fast-paced healthcare environment.
  • Knowledge of healthcare laws regulations and best practices.
  • Excellent organizational and time management skills.
  • Critical thinking and problem-solving abilities.
