Overview

Shift: Saturday - Sunday 8:00 AM - 8:00 PM ( rotating call)

As a Registered Nurse Hospice youll bring compassion clinical expertise and a deep sense of purpose to patients and families during lifes most important moments. Working in the comfort of each patients home youll perform physical assessments deliver personalized care and provide critical education and support. Collaborating closely with physicians and the care team youll help create and carry out individualized hospice care plans that prioritize comfort dignity and quality of life. Responsibilities

Makes visits to the homes of patients requiring hospice nursing services and assumes responsibility for an ongoing interdisciplinary assessment of the patient.

Ensures compassionate empathetic quality and safe delivery of services to patients according to care plan. Uses equipment and supplies safely effectively and efficiently.

Consistently utilizes a patient-centered approach considering physical psych/social spiritual educational safety and related criteria appropriate to the age of the patients served in hospice.

Delivers quality care and completes clear concise and accurate clinical documentation of care provision in accordance with hospice conditions of participation (CoPs) various payer hospice coverage criteria accreditation standards professional standards for discipline/credential and the hospice plan of care.

Maintains accurate and timely documentation of clinical records and time/travel entries.

Qualifications

Education:

  • Graduate of State Board approved program for Registered Nurses

Licensure/Certifications:

  • Valid license as an RN in state(s) where providing care
  • Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.
  • Mandatory Reporter: Child & Dependent Adult Abuse.
  • CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.

Minimum Experience:

  • One year nursing experience caring for similar patient population as to be assigned

Other:

  • Use of usual and customary equipment used to perform essential functions of the position.
