Position Summary: The Outreach Specialist HONEY (Housing our Newborns Empowering You) is accountable directly to the Program Director HONEY. This is a full-time position based out of the Virginia Williams Family Resource Center and the COH Family Health and Birth Center locations in NE DC.


The pay rate for this position ranges between $26.44 - $28.84 per hour and the offer amount is determined by the candidates education qualifications and experience . Indeed provides their own estimated salary calculator and is not affiliated with COHs range.


Highlighted Duties and Responsibilities:

Patient Care:

  • Works as part of an integrated care team at the Virginia Williams Family Resource Center to connect people who are experiencing homelessness and are pregnant to perinatal care and support systems.
  • During initial intake assesses clients connections or barriers to receiving perinatal services preferences for care risk factors and works to opt in client to HONEY perinatal care coordination.
  • Identifies and links participants to appropriate and preferred community resources perinatal care and wraparound supports for family in coordination with assigned housing case manager.
  • Opts in schedules and sends rides for patients through Rides for Moms partnership with Uber Health ( 30 rides per month).
  • Document any relevant patient encounters in eCW within 3 business days of interaction. Updates referral and client tracking databases within 1 business day of interaction with clients
  • Secures patient information and maintains patient confidence by completing and safeguarding medical records and keeping patient information confidential.

Outreach & Education:

  • Conducts outreach activities to develop relationships with HONEY program participants and partnerships with other government and community-based organizations.
  • Conducts outreach to homeless service and healthcare providers to build partnerships and opportunities for collaboration training and client support (e.g.: referrals programming updates etc.).
  • Attends community events tabling events meetings and conferences as requested to promote the HONEY program to potential clients and partners.
  • Educates partners clinicians and housing system staff on issues of perinatal health to support families during pregnancy and infancy.
  • Conducts program overviews and educational sessions virtually in person and asynchronously for healthcare and homeless service providers to better support their work with pregnant people experiencing homelessness (approximately 2-4 sessions per month).
  • Consults with COH Maternal & Child Health team and Housing team members in the development of presentations and training materials
  • Creates pre- and post-test tools to assess the effectiveness of educational sessions modifying and creating new training resources based on pre- and post-test data
  • Performs other duties as assigned by supervisor.
Requirements
  • Bachelors Degree Required.
  • Prior instructional experience required; experience in adult education social services health care or nursing preferred.
  • Experience working with under resourced populations including pregnant people experiencing homelessness preferred.
  • Strong organizational written and verbal communication skills.
  • Strong interpersonal skills and comfortability in conducting cold calls and outreach.
  • Basic graphic design marketing and presentation skills required.
  • Must have reliable transportation.
  • In relation to remote work versus on-site expectations this position is classified as the following: Hybrid: Remote work and on-site work requirements are generally split roughly 60% on-site/40% remote. This position will be based primarily at Virginia Williams Family Resource Center (at 64 New York Avenue NE) with on-site expectations of three days a week of which one day will be Monday. Tour of duty hours are 8:30am-4:30pm (or 9:00am-5:00pm). Please note that remote work designations are subject to change or fluctuate at any point in time and the supervisor may require in person learning for a specific amount of time after hire

About Us:

Community of Hope is a mission-driven innovative and rapidly growing nonprofit. For 45 years we have provided healthcare housing and community support services to make Washington DC more equitable. As a Federally Qualified Health Center we provide medical dental emotional wellness and care coordination services for the whole family at three locations in DC. Community of Hope also strongly emphasizes maternal and child health with midwifery practice and the only free-standing birth center in 2024 Community of Hope provided about 50000 medical visits 6300 dental visits and 17000 behavioral health visits for about 16000 patients. Community of Hope is also one of the largest providers of homelessness prevention and housing services for 1600 households1384 families and 220 individualsexperiencing homelessness in DC. Community support through Family Success Center our WIC nutrition centers and perinatal care coordination for pregnant people experiencing homelessness reach hundreds more families.
