Below mentioned is the job description for your reference:
Job Title: Sr. LTSS Service Care Manager RN
Duration: 6 months contract
Shift: 8 am 5pm may require after hour visits as per member request
Location: El Paso TX
Responsibilities:
Complete in-home assessment which includes:
o SAI Core A to Core Q
o ISP
o EDP
o Other forms as applicable: 1500 forms SBP EVV
o Assessing the members current health status resource utilization past and present treatment plan and services short- and long-term goals treatment and provider options.
o Follow up on requests for items/services
o Speaking to providers
Provide approximately 14 weeks of training which includes Core Training and working with a tenured CM; prior to the new hire completing a full week of visits (5 visits per week)

Required ADN or BSN
Licensure
Required: RN
Skills:
Registered Nurse 4 years of RN experience required
Pediatric Experience
Bilingual
