Nebraska Medicine is seeking a Kidney Transplant Educator to join the Transplant Team located in Omaha NE. The Transplant Educator provides comprehensive education and support to transplant patients and their families helping them understand their condition treatment plan and long-term care needs. This role promotes self-management strategies that empower patients to achieve individualized goals and optimize health outcomes across the continuum of care. The educator collaborates with the transplant team to integrate education into the plan of care develops and delivers patient- and community-focused educational programs and serves as a resource and advocate for patients navigating complex medical information.

Monday - Friday 8AM - 5PM

Provide education and support for the transplant patient population by helping them to better understand and manage their condition. Encourage self-management that promotes the achievement of individualized behavioral and treatment goals that optimize health outcomes for transplant patients across the health care continuum. Provide educational opportunities to the community about transplant.
