Job Description

Basic Function: Basic Function: Under the direction of the HIV and Hep C Project Director the HIV Community Health Outreach Worker (CHW) will be responsible for providing outreach peer support and education services within the scope of given projects to meet the program requirements of certain grants awarded to Canyonlands Healthcare and other projects as appropriate. The position requires strong interpersonal skills to make connections and build relationships with the target patient/client base. The Community Health Worker will work collaboratively with other departments at CHC. CHW will also work alongside Community-based agencies medical providers primary teams to assist in coordinating care with the purpose of making critical linkages to health information and services for community members to improve patient care and outcomes.

Requirements

Essential Functions:
Work closely with the Pharmacy Director and Grant Manager in tailoring health education and remote patient monitoring plans for each client in the cohort.

  1. Provide health education in the clients least restrictive environment to include and not be limited to home visits.
  2. Promotes the Remote Patient Monitoring Program during local health fairs cultural events and community meetings.
  3. Contacts patients in person by phone or in writing to ensure they have completed recommended actions. Monitors patients to evaluate proper use of remote monitoring equipment and ensures reported results provide a true and accurate representation of the patients condition.
  4. Maintains log of all patient contact. All telephone contacts are logged in the patients EHR.
  5. Interpret translate and provide cultural mediation related to remote health monitoring for community members.
  6. Collaborates with the health care team responsible for patient care to implement and monitor appropriate clinical care and other health care services to meet the needs of the patient. Provides technical assistance and consultations to staff and family.
  7. Coordinates in the development and review of clinical pathway trends and shares with appropriate service and management teams.
  8. Assists with monitoring of quality improvement elements including establishment of program measures and implementation of action plans.
  9. Reviews and understands strategies to assist patients with accessing appropriate levels of care.
  10. Provides clients with self-management skills and educational information needed so they can be full participants in their own treatment and care.

Minimum Qualifications:

  1. Ability to perform each essential function duty satisfactorily with or without reasonable accommodation. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
  2. High School Diploma or equivalency.
  3. Previous experience working in a medical setting as an active team member.
  4. Good interpersonal and human relations skills are required. Must possess skills to empathize and care for a variety of patients including low-income and diverse cultures and treat everyone (internal and external customers) with respect and dignity.
  5. Has a working knowledge of medical health conditions.
  6. Has strong communication skills.
  7. Has the ability to help design and implement clinical pathways and protocols for treatment of selected chronic conditions.
  8. Ability to maintain confidentiality at all times in all situations and follow the laws and intent of the most current HIPAA laws and regulations and Canyonlands Healthcare security and privacy policies.
  9. CPR certified (BLS).
  10. Possess a valid drivers license.
  11. Ability to read write and speak English.
