Transition Home Healthcare is an elite home health agency serving Houston TX and surrounding areas since 2010. Speech Language Pathologist
Shifts Available:PRN
Service Areas with Openings: Southwest & Southeast
Responsibilities:
- Facilitates rehabilitation of patients with emotional physical or mental disabilities.
- Implements speech therapy care plans.
- Coordinates care with entire team.
- Educates patients and family members.
- Prepares timely complete and accurate clinical notes.
- Communicates effectively with patients caregivers healthcare providers office support and other care team members.
Qualifications:
- Speech language pathologist in the State of Texas.
- Home health experience highly preferred.
- Experience with Home Care Home Base EMR (preferred but not required).
- Knowledge of home health regulations and general healthcare compliance.
- Ability to handle physical workload.
- Good driving record.
- Proficient level experience in Microsoft Office (Word Excel PowerPoint Outlook).
- Exceptional communication and people skills.
