3-month contract with a local authority
Job Purpose
- To provide early help and targeted support to vulnerable children young people and their families preventing escalation to statutory social care services. The role focuses on delivering the right help at the right time in line with the Supporting Families Agenda by conducting holistic family assessments developing personalized plans and coordinating interventions with partner agencies to achieve positive and lasting outcomes.
Key Duties/Accountabilities
- Undertake comprehensive holistic family assessments to identify strengths needs and risks.
- Work collaboratively with families to develop tailored outcome-focused support plans.
- Coordinate and monitor targeted interventions ensuring support is appropriate and effective.
- Facilitate and chair Team Around the Child (TAC) meetings to review progress and adapt plans.
- Build positive trusting relationships with children young people parents and carers.
- Work in partnership with statutory and voluntary sector agencies to deliver joined-up services.
- Maintain accurate timely case records in line with service policies and confidentiality requirements.
Essential Experience Required
- Proven experience working directly with children young people and families preferably in an early help or targeted support setting.
- Experience of completing holistic assessments and creating personalized outcome-based plans.
Essential Qualification Required
- Level 3 qualification (or higher) in a relevant field such as Social Care Youth Work Education Early Years or Health.
- Social Work England Registration

- Enhanced DBS with barred list is required for the role.
