You will be updated with latest job alerts via emailMeet with service users carers families and professionals in various community settings (homes clinics care settings) to conduct functional assessments and identify goals.
Promote independence rehabilitation and wellbeing through person-centred planning equipment provision and home/environmental adaptations.
Develop and implement individual care and support plans including strategies for rehabilitation and risk reduction.
Coordinate assessments and contribute to multi-agency case conferences including preparing and presenting reports to panels (e.g. housing funding safeguarding).
Specify and recommend assistive equipment and minor/major adaptations to promote safety and functional ability.
Monitor and review intervention plans and adjust as necessary.
Work in accordance with relevant legislation including the Care Act 2014 Housing Grants and Health and Safety frameworks.
Maintain accurate clear and timely records of assessments decisions and interventions in line with professional and council standards.
Adhere to the Royal College of Occupational Therapists Code of Ethics and HCPC Standards of Proficiency.
Take responsibility for managing own workload meeting performance targets and deadlines and escalating complex cases or workload issues to the Team Manager.
Degree/Diploma in Occupational Therapy.
HCPC registration as an Occupational Therapist.
Experience working in community or social care settings with adults.
Knowledge of relevant legislation including Care Act Mental Capacity Act and housing/adaptation policies.
Skilled in functional assessment rehabilitation planning and risk assessment.
Strong knowledge of equipment provision and home adaptations (e.g. Disabled Facilities Grants).
Effective written and verbal communication skills.
Ability to work collaboratively with multi-agency teams and independently manage a caseload.
Understanding of safeguarding responsibilities and data protection legislation.
