General Summary:
- In collaboration with Psychiatric Urgent Care (PUC) clinical providers coordinates linkages to care for urgent care patients.
- Patients include children adolescents adults and older adults. Data collection and patient registration.
- Coordinates with Admission Day Hospital Care Connect and other programs.
- Communicates with patients/families and with clinical providers.
- Functions as liaison between community referral sources and Psychiatric Urgent Care providers.
Knowledge Skills and Abilities Required:
- Work requires knowledge of psychiatric disorders and diagnosis as acquired through a Bachelors degree in Psychology Social Work Psychiatric Rehabilitation or related field of study.
- For internal candidates high school diploma plus an additional 3 year of relevant internal experience may be considered as a substitute for a bachelors degree.
- Position requires computer skills and ability to learn new computer programs.
- Work requires interpersonal skills to communicate effectively with patients families referral sources and co-workers sometimes under stressful circumstances (51-80% of work time). Urgent Care patients and their families may be stressed anxious irritable and/or distraught.
