Minimum Qualifications
- Masters degree in social work or related field
- 2yrs Social Work professional counseling or marriage/family therapist counseling
License Required
- LCSW Licensed Clinical Social Worker (TBHEC) OR
- LPC - Licensed Prof Counselor (TBHEC) OR
- LMFT - Licensed Marriage &Family Therapist (TBHEC)
Preferred
- employment in healthcare a health care specialization in schooling or a field work practicum in health care
Knowledge and Skills
- Knowledge of evidence-based psychosocial models theories and interventions; medical social work; and community resources.
- Ability to provide evidence-based assessment and therapeutic interventions tailored to the patients presenting symptoms and goals.
- Ability to conduct a thorough psychosocial assessment and develop implement and evaluate a plan of care.
- Ability to conceptualize the psychosocial needs presented by the assigned patient population to intervene with patient/family behavior to coordinate community resources and to handle crises and complex psychosocial situations.
- Ability to handle grief.
- Ability to demonstrate initiative good judgment and problem-solving skills.
- Ability to demonstrate excellent professional listening and communication skills.
Specific Duties
- Provides psychosocial screenings/assessments by interviewing patients and/or their families for information about the patients social emotional/psychological developmental/educational/vocational environmental financial and health situations within authorization periods as required for patient care and/or treatment/transitional planning.
- Provides evidence-based psychotherapy for individual family parent and groups to help patients/families reduce symptoms of mental illness and improve functioning across environments Establishes achievable treatment goals with patients.
- Tracks patient progress via ongoing symptom and functional assessment using reliable and valid measures where available and adjusts treatment plan in accordance with findings
- Coordinates care with other mental health providers to insure selected treatments match the patients current diagnosis symptoms developmental level and functioning across environments.
- Actively participates in providers conferences to identify and respond to the psychosocial needs of the patients/families and to provide psychosocial information pertinent to formulating Plans of Care.
- Assesses and follows-up with providers recommendations of changes in the patient/family status and/or needs as appropriate.
- Conducts patient/family needs assessments identifies and assesses patients/families knowledge and utilization of community resources and provides information about additional appropriate community resources.
- Assesses patients/families understanding of the referral information provided and their ability to access the recommended resources.
- Demonstrates awareness of required competencies for provision of evidence-based assessment therapy services specific to the population and setting where services are to be provided.
- Seeks specific consultation supervision didactic training or education in areas of competency to be developed.
