Job Description
Duties:
Provides professional Nursing Service such as assessment for psychiatric and medical problems identified for all patients admitted to the Assertive Community Treatment Provides and maintain a therapeutic milieu conductive to mental health/healing in the community. Participates in group dynamic sessions and multidisciplinary team conferences. ages - Adolescent Adult Geriatric 12 - 21 yrs. 22 - 65 yrs. 65 yrs. Will the nurse be reporting to facility first and then receiving their assigned cases Team meeting is held every day at 3 pm to plan the next days visits. So initially the nurse will report to the facility and meet with the team and team leader. Going forward will know the schedule for each day How many cases/patients a day This can vary day to day depending on if the visit includes administering long acting injectable antipsychotic but no more than six in a day Will they travel to patient homes within just Bronx or other boroughs Bronx only Will they have to provide their own transportation or Jacobi will be providing The team has vehicles the staff use. Will not have to provide their own transportation.Skills:
- Two years of psych experience is required
Education: Required
- New York State RN License
- Primary Source Verification
- AHA BLS
- CPI certified
