Description

The Safe Journey Case Manager (SJCM) under the leadership of the Director of Program Operations (DPO) Teen Parenting Services provides wraparound services for teen parents and their children age birth-5 as an intervention to prevent child abuse and neglect. The primary goal is to assist teen parents with high school completion and delaying additional pregnancies. The ideal candidate will have experience with case management completing assessments and evaluations conducting home visits and linking students with tutors and other needed interventions as identified. This position is also responsible for entering documentation reports and other necessary correspondences in a timely manner. Strong interpersonal skills to collaborate with other youth and human service agencies staff and service teams within the community are needed. Safe Journey Case Managers provide parent education services facilitate the Parents as Teachers (PAT) parenting curriculum and ensure participants have transporting to and from program events according to program guidelines. Additional duties include but are not limited to connecting children and families with needed services as an intervention to promote education and healthier lifestyles managing childcare slots assigned to caseload participants and providing assistance as needed to teammates assigned to specific events and/or projects.


ESSENTIAL DUTIES AND RESPONSIBILITIES

Program Duties:

  • Provide case management assessments and evaluation services to 20 teen parents and their children age birth-5.
  • Monitor support and provide services to target parents that assist them with staying in school and completing their high school and post-secondary education.
  • Help students recognize their academic strengths and weaknesses by linking participants with tutors and other specific interventions as needed and acting as advocates on the teens behalf to assist with promotion and or graduation.
  • Work in concert with the schools social workers nurses school psychologists and student services teams to increase students academic and attendance concerns; ensuring that students meet their academic potential.
  • Educate parents about effective social health and employment support systems.
  • Coordinate and assist participants with healthcare and immunization needs.
  • Facilitate group and individual parenting sessions in order to educate participants about appropriate child developmental issues.
  • Collaborate with other youth/human service agencies staff and service teams in the community to provide needed services to participants.
  • Help support Communities In Schools (CIS) special events and collaborate with other CIS staff at school site.

Volunteer Administration:

  • Recruit volunteers/mentors (as needed) and coordinate application process with CIS Volunteer Coordinator. Assist with providing program specific volunteer training and special recognition for volunteers.
  • Communicate volunteer needs to Director of Program Operations (as needed).
  • Work with program staff to recognize volunteers and their contributions.

Documentation and Evaluation:

  • Document and file all services and related correspondences in the Penelope database and file in charts within three days of the designated service.
  • Generate case notes reports and other required correspondences monthly.
  • Provide accurate and quality documentation of services contacts with students and number of participants served on this caseload.
  • Maintain up-to-date files on all participants.
  • Assist with quarterly and end of year reporting as needed.

Working Relationships:

  • Build and maintain appropriate relationship with Human Service and collaborative agencies making appropriate referrals as deemed necessary.
  • Maintain a high level of communication and relationship-building with internal and external collaborative partners and community stakeholders in an effort to improve services to teen parents.
  • Participate with Charlotte-Mecklenburg School (CMS) staff in Student Services team meetings and staffing at designated schools regarding participants on their caseload.
  • Work effectively with other human service providers and department staff to ensure smooth coordination of consumer services. When establishing new services or working to solve problems with existing service delivery determine who when and how to seek assistance from within partnering agencies with the intent of resolving problems or concerns while maintaining strong professional relationships with partners.


QUALIFICATIONS

To perform this job successfully an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge skill and/or ability required.


EDUCATION and/or EXPERIENCE

  • Bachelor Degree in counseling human service social work education or another related field
  • Minimum of two years experience working successfully with adolescents and evidence of working within collaborative service teams
  • Ability to work some evenings and weekends as needed

SKILLS REQUIREMENT

  • Demonstrated computer proficiency in Microsoft Office experience with Google Suite Zoom or other virtual meeting platforms
  • Excellent communication (verbal and written) interpersonal and people management skills
  • Ability to cross cultural boundaries to build and maintain strong working relationships for organizational success
  • Strong problem solving skill with the ability to work through tough issues quickly and effectively and maintain positive relations
  • Sensitivity and ability to appreciate the points of view of personnel within the schools other agencies/organizations including a keen awareness of their needs and how to build collaborative partnerships based upon fundamental CIS principles.
  • Awareness of cultural dilemmas and pressures that affects poverty
  • Ability to be flexible in a work environment often filled with ambiguous situations
  • Must be self-motivated well organized detail oriented and able to effectively work independently and within a team framework
  • Ability to work a flexible schedule

PHYSICAL DEMANDS

While performing the duties of this job the employee is frequently required to sit; use hands to handle feel and keyboard; reach with hands and arms; and talk or hear. The employee is occasionally required to stand; walk; stoop kneel or bend. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close and distance vision.

WORK ENVIRONMENT

The position is located within the Charlotte Mecklenburg area. Regular and satisfactory attendance and punctuality are required. Regular and satisfactory performance including meeting all process deadlines and stated deliverables is required.
