OCES is hiring an Options Counselor to support our mission of providing support to elders and adults with disabilities with vital information on community-based long term care services and supports promoting healthy and safe lives for our consumers in their communities.
What you bring:
- Bachelors degree in Social Work or related field; current social work license preferred
- Passion for helping others and the dedication to make a difference in peoples lives.
- Two years experience in a human services environment.
- Working knowledge of community services and supports
- Ability to work effectively with consumers families staff providers and the general public
- Your ability to express yourself clearly both verbally and in writing
What you will do:
- Provide counseling regarding community-based services and supports to consumers
- Discuss and provide guidance regarding
- Benefits and supports available to meet long term care needs
- Factors in making care decisions considering consumers preferences
- Assist consumers in developing plans that are consumer-directed and include appropriate services programs financial resources and other supports
- Follow-up by communicating and consulting with consumers as needed
- Complete and maintain documentation
