For the traditional case management portion the candidate will work under the direction of a case management supervisor assessing clients case management needs and goals completing treatment planning with clients to address their identified needs and goals providing referrals and links to community services and resources reflective of the treatment plan as well as monitoring service delivery and evaluating the effectiveness of resources and service provisions. For the Supportive Housing Specialist portion the candidate will work under the supervision of a case management supervisor following a Housing First model with SMI and non-SMI residents. The candidate must be flexible and self-motivated able to work both independently and be effective with multi agency communication within the Housing First Model supportive apartment complex.
