Corewell Health South is seeking a compassionate patient centered Nurse Practitioner to join our dedicated team of providers to care for hospice patients in our community. At Corewell Health Hospice when considering end-of- life care we focus on making every day count and embrace the best quality of life possible. Our team is there to help manage pain provide comfort and offer support.
Responsible for managing patients and coordinating the services provided by multiple disciplines within and outside the health system. Maintains a collaborative practice agreement with specific physician(s). Exercises independent judgment in the assessment diagnosis and initiation of selected protocols/procedures. Provides educational support for patients/significant others health team members and the community and coordinates QA/QI studies to measure outcomes of care.
